Life Insurance

Life insurance will help financially protect you and your family in the case of death or serious injury.

Basic Term Life coverage is paid for by OHM Advisors and coverage is insured by Unum as follows:

Basic Term Life
Eligible Employee/Class	BENEFIT
Full-Time Employees	2x annual earnings up to $100,000
In addition to providing Life coverage for you, OHM Advisors also pays for Life coverage for your spouse and dependent children as follows:
DEPENDENT	BENEFIT
Spouse	$5,000
Child (live birth to 6 months)	$1,000
Child (6 months to age 19, or 26 with full-time student status)	$2,000

Optional Term Life coverage is available for employees to purchase and is also insured by Unum as follows:

	Optional Term Life Benefit	When is Evidence of Insurability Required
Employee	$10,000 increments to the lesser of 5X your earnings or $300,000.	- If you are electing more than $200,000. - After this true open enrollment, you will need EOI if you waived coverage, and choose to elect coverage later.
Spouse	$5,000 increments to the lesser of 100% of your benefit amount or $300,000.	- If you are electing more than $25,000 for your spouse. - After this true open enrollment, you will need EOI if you waived coverage for your spouse, and elect coverage later.
Child	$2,000 increments to the lesser of 100% of your benefit amount or $10,000.	- EOI is not required for children.
Delayed Effective Date	If your spouse or child has a serious injury, sickness, or disorder, or is confined, their coverage may not take effect . Refer to the plan booklet for details.

If you need to update your beneficiary information, you will need to submit a new form to the OHM Advisors Human Resource Department.
