Family and Medical Leave Act (FMLA)
OHM Advisors will provide up to 12 weeks of unpaid, job-protected leave to “eligible” employees for certain family and medical reasons (or 26 weeks of unpaid military caregiver leave).
The ultimate determination of whether an employee is entitled to FMLA leave or whether any time off by the employee may be deemed to be FMLA leave is governed by the provisions of FMLA, as well as the applicable regulations and other law interpreting FMLA. Also, OHM Advisors will comply with state or local laws or regulations that provide greater leave or related entitlements to employees.
Unum will serve as your administrator on behalf of OHM Advisors to manage leaves under the Family and Medical Leave Act (FMLA), state leave laws, corporate leave policies, and short-term disability claims. If you are currently on an approved FML or STD leave, there will be no changes to your approval. If you have any questions about a pending leave request, please contact UNUM at (866) 779-1054.
How to apply for FMLA, Paid Sick Leave and Short-Term Disability
- To request a new Family Medical Leave, Paid Sick Leave or Short-Term Disability, you must contact UNUM to initiate your request.
- If you have any FMLA leave-related questions, you must call UNUM.
- For UNUM to decide whether to approve your new leave request, you must submit a Certification for Leave and Disability form.
- To report time off for intermittent leave, you must contact UNUM. For continuous multiple days or further notice FMLA absences, you will work with UNUM for the absence duration.
- To be cleared to return to work from an FMLA leave, you must follow UNUM’s Return to Work (RTW) process. If a Fitness for Duty form or physician’s note is required, you will be advised before returning to work.
